Job Description
CVS Health is seeking a dedicated and compassionate Telemedicine Health Coach to join our dynamic team. As a Telemedicine Health Coach, you will play a crucial role in providing remote health coaching and support to our diverse patient population. You will assist patients in achieving their health and wellness goals through personalized coaching sessions conducted via phone or video conferencing. This position offers the flexibility of working remotely from the comfort of your home while contributing to the wellbeing of our patients.
Responsibilities
Conduct telemedicine health coaching sessions with patients using phone or video conferencing tools.
Develop personalized coaching plans to help patients achieve their health and wellness goals.
Monitor patients progress and provide ongoing support and motivation.
Educate patients on lifestyle modifications, such as nutrition, exercise, stress management, and medication adherence.
Collaborate with healthcare providers to ensure a coordinated approach to patient care.
Maintain accurate and confidential patient records in accordance with HIPAA regulations.
Stay current with industry trends and best practices in telemedicine and health coaching.
Requirements
Bachelors degree in a health-related field (such as Nutrition, Exercise Science, Nursing, or Public Health).
Certification in health coaching (such as NBC-HWC) or a related field.
Minimum of 2 years of experience in health coaching or a similar role.
Strong understanding of chronic disease management and preventative care.
Excellent communication and interpersonal skills.
Proficient in using telehealth platforms and technology.
Ability to work independently and manage time effectively.
Educational Qualifications
Bachelors degree in a health-related field (such as Nutrition, Exercise Science, Nursing, or Public Health).
Certification in health coaching (such as NBC-HWC) or a related field is highly preferred.
Experience
Minimum of 2 years of experience in health coaching or a similar role.
Experience with telehealth platforms and remote patient engagement is preferred.
