In this role, you will utilize your nursing expertise and passion for health and wellness to guide and support patients in managing their health conditions and achieving their wellness goals. You will work collaboratively with a diverse team of healthcare professionals, ensuring that our members receive comprehensive, personalized care tailored to their needs.
**Requirements**
– Active RN license in good standing
– Minimum of 2 years of clinical nursing experience
– Strong communication and interpersonal skills
– Proficiency in using telehealth platforms and electronic health records (EHR)
– Ability to work independently and manage time effectively
– Strong critical thinking and problem-solving abilities
– Commitment to maintaining patient confidentiality and adhering to HIPAA regulations
**Responsibilities**
– Provide one-on-one health coaching and support to members via phone and video consultations
– Develop personalized health plans based on individual assessments and evidence-based practices
– Educate members on managing chronic conditions, medication adherence, and lifestyle modifications
– Collaborate with other healthcare professionals to coordinate care and ensure comprehensive support
– Monitor member progress, adjust care plans as needed, and provide ongoing encouragement and motivation
– Document all interactions and care plans accurately in the EHR system
– Participate in continuous education and training to stay current with best practices and industry standards

**Educational Qualifications**
– Bachelor of Science in Nursing (BSN) preferred, but Associate Degree in Nursing (ADN) considered with relevant experience
– Certified Health Coach (CHC) or similar certification preferred
**Experience**
– Minimum of 2 years of clinical nursing experience
– Experience in health coaching, telehealth, or case management is highly desirable
