Description
CVS Health is seeking a compassionate and experienced Telephonic Health Nurse to join our dynamic team. This position provides a unique opportunity to work remotely while contributing to the health and wellness of our members. As a Telephonic Health Nurse, you will play a critical role in managing patients’ health through telehealth services, providing education, support, and clinical assessments over the phone.

Requirements
1. **Registered Nurse (RN) license**: Must be active and in good standing.
2. **Telehealth Experience**: Previous experience in telehealth or telephone triage preferred.
3. **Clinical Experience**: Minimum of 3 years of clinical nursing experience.
4. **Communication Skills**: Excellent verbal and written communication skills.
5. **Technology Proficient**: Comfortable using telehealth platforms, electronic health records (EHRs), and other digital tools.
6. **Time Management**: Ability to manage time effectively and handle multiple tasks simultaneously.
7. **Patient-Centric**: Strong commitment to patient care and improving health outcomes.
Responsibilities
1. Conduct telephonic assessments and triage for patients, providing medical advice and support.
2. Educate patients on health conditions, treatment plans, and medication management.
3. Monitor patient progress and follow up to ensure compliance with care plans.
4. Collaborate with healthcare providers and other team members to coordinate patient care.
5. Document patient interactions accurately in the electronic health record (EHR) system.
6. Provide emotional support and counseling to patients and their families as needed.
7. Participate in ongoing training and professional development opportunities to stay current on best practices in telehealth nursing.

Educational Qualifications
– Bachelor’s Degree in Nursing (BSN) preferred, but Associate Degree in Nursing (ADN) will be considered.
– Telehealth certification is a plus.
Experience
– Minimum of 3 years of clinical nursing experience required.
– Previous experience in telehealth or telephone triage is highly desirable.
