Requirements
Active and unrestricted RN license in your state of residence
Minimum of 2 years of nursing experience, preferably in a clinical setting
Experience in case management or care coordination is a plus
Strong communication and interpersonal skills
Proficiency in using electronic medical records and telehealth platforms
Ability to work independently and as part of a remote team
Reliable internet connection and a quiet, private workspace
Responsibilities
Conduct telephonic assessments to evaluate patients’ health status and needs
Develop and implement personalized care plans based on patients’ health goals
Provide education and resources to patients and caregivers on managing chronic conditions
Coordinate with healthcare providers to ensure continuity of care
Monitor patients’ progress and adjust care plans as needed
Maintain accurate and timely documentation in electronic medical records
Participate in team meetings and professional development opportunities
