As a Telehealth Nurse Coordinator, you will be responsible for coordinating and delivering comprehensive health services through telehealth platforms. This involves patient education, triage, care coordination, and support across various health conditions. You will work closely with multidisciplinary teams to ensure patients receive optimal care and resources.
Key Responsibilities
– Provide telehealth consultations to patients, addressing their health concerns and providing medical advice.
– Coordinate patient care plans and follow-ups in collaboration with healthcare providers.
– Conduct patient assessments, including reviewing medical histories and current health conditions.
– Educate patients on disease management, treatment options, and preventive health measures.
– Ensure accurate and timely documentation of patient interactions and care plans in the electronic health record (EHR) system.
– Monitor patient progress and adjust care plans as necessary to meet patient needs.
– Maintain up-to-date knowledge of telehealth best practices, protocols, and regulations.
Requirements
– Active Registered Nurse (RN) license in the state of practice.
– Minimum of 2 years of clinical nursing experience.
– Prior experience in telehealth or remote patient care is highly desirable.
– Proficiency in using electronic health record (EHR) systems.
– Strong communication and interpersonal skills.
– Ability to work independently and manage time effectively.
– CPR certification (BLS) is preferred.
Educational Qualifications
– Bachelor of Science in Nursing (BSN) required; Masters degree in Nursing or related field is a plus.
