Requirements
Minimum Qualifications:
Active RN license in the state of residence.
Bachelors degree in Nursing (BSN) preferred.
Minimum of 2 years of clinical nursing experience.
Experience in disease management or chronic care management is a plus.
Proficiency in using telehealth platforms and technology.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Educational Qualifications
Bachelors degree in Nursing (BSN) preferred.
Active RN license in the state of residence.
