CVS Health is seeking a dedicated and compassionate Remote RN Case Manager to join our dynamic team. In this role, you will leverage your nursing expertise to provide comprehensive case management services to our members. This position allows you to work from the comfort of your home while making a significant impact on the lives of individuals by coordinating their healthcare needs and ensuring they receive the right care at the right time.
As a Remote RN Case Manager, you will be responsible for assessing, planning, and coordinating care for our members, promoting quality outcomes, and optimizing member benefits. You will work collaboratively with members, their families, and a multidisciplinary team to ensure a seamless healthcare experience.
Requirements
– Active and unrestricted Registered Nurse (RN) license in the state of residence
– Minimum of 3 years of clinical experience in nursing
– Experience in case management, discharge planning, or home health is preferred
– Proficiency in using electronic medical records and case management software
– Excellent communication, organizational, and time management skills
– Ability to work independently and as part of a team
– Knowledge of healthcare regulations and compliance standards
Responsibilities
– Conduct comprehensive assessments of members’ health status, medical history, and healthcare needs
– Develop individualized care plans that address the specific needs and goals of each member
– Coordinate and facilitate communication between members, their families, and healthcare providers
– Monitor and evaluate the effectiveness of care plans and make necessary adjustments
– Educate members and their families about their health conditions, treatment options, and available resources
– Advocate for members to ensure they receive appropriate and timely care
– Maintain accurate and up-to-date documentation in accordance with company policies and regulatory requirements

Educational Qualifications
– Bachelor of Science in Nursing (BSN) preferred
– Certification in Case Management (CCM) is a plus
