Requirements
Active Registered Nurse (RN) license in the state of residence and ability to obtain licensure in other states as required.
Minimum of 3 years of clinical experience.
Strong communication and interpersonal skills.
Proficient in using electronic medical records (EMR) and telehealth platforms.
Ability to work independently and as part of a multidisciplinary team.
Excellent organizational skills and attention to detail.
Comfortable with technology and virtual communication tools.
Responsibilities
Provide telehealth nursing services to patients, including assessment, diagnosis, and treatment planning.
Educate patients and their families on disease management, treatment options, and preventive care.
Collaborate with healthcare providers across various disciplines to ensure comprehensive patient care.
Document patient interactions and care plans accurately in the EMR system.
Participate in continuous quality improvement initiatives and professional development activities.
Maintain patient confidentiality and adhere to HIPAA regulations.
Support patients in navigating the healthcare system and accessing necessary resources.
Educational Qualifications
Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience considered.
Current certification in Basic Life Support (BLS).
Advanced certifications (e.g., CDE, CCM) are a plus.
Experience
Minimum of 3 years of clinical nursing experience.
Experience in telehealth or remote patient monitoring preferred.
