You will be responsible for assessing patients health conditions, developing care plans, and providing education and support to patients and their families.
Requirements
1. Active and unrestricted RN license.
2. Minimum of 3 years of clinical nursing experience.
3. Excellent communication and interpersonal skills.
4. Proficiency in electronic health records (EHR) software.
5. Ability to work independently and manage time effectively.
6. Reliable high-speed internet connection.
7. Strong problem-solving skills and attention to detail.
Responsibilities
1. Conduct telehealth consultations with patients.
2. Assess patient health status and provide appropriate treatment recommendations.
3. Collaborate with healthcare team members to develop and implement patient care plans.
4. Provide patient education on disease management and preventive care.
5. Document patient interactions and maintain accurate medical records.
6. Monitor patient progress and adjust care plans as necessary.
7. Participate in continuous professional development and training programs.

Educational Qualifications
– Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN) from an accredited institution.
– Current Basic Life Support (BLS) certification.
Experience
– Minimum of 3 years of clinical experience in a healthcare setting, preferably in a remote or telehealth role.
– Experience with chronic disease management and patient education is highly desirable.
