Job Summary We are seeking a part-time remote Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality patient care and supporting our team of healthcare professionals. Key Responsibilities As a Licensed Practical Nurse (LPN) at UMass Memorial Health, you will be responsible for: Assessing the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, taking and monitoring vitals of clients on the unit, medication administration, and documentation. Renders and supervises care of clients on the unit. Monitors medication and consultation with prescribers. Providing appropriate health care and coordinating referrals as prescribed by providers. Participating in the evaluation of client’s condition and progress with team members. Promoting education and adherence to prescribed treatment protocols. Facilitating wellness and medically related psychoeducational groups to individuals. Recording all client information within the electronic medical record system daily. Assisting RN on a daily basis. Offering nursing consult on emergency service evaluations. Working with clients to coordinate care with their PCP, psychiatrist, and other professionals, including but not limited to medication refills and follow-up medical appointments. Standard Staffing Level Responsibilities In addition to the key responsibilities listed above, you will also be expected to: Comply with established departmental policies, procedures, and objectives. Attend a variety of meetings, conferences, seminars as required or directed. Demonstrate use of Quality Improvement in daily operations. Comply with all health and safety regulations and requirements. Respect diverse views and approaches, demonstrate Standards of Respect, and contribute to creating and maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors. Maintain regular, reliable, and predictable attendance. Perform other similar and related duties as required or directed. Requirements and Qualifications To be considered for this exciting opportunity, you must: License/Certification/Education Be a graduate of an accredited school of Nursing with a current license as a Licensed Practical Nurse in the state of Massachusetts. Have a satisfactory CORI background check. Note: Driving is not a requirement for this position. Experience/Skills Demonstrate proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies. Demonstrate the ability to communicate effectively and professionally with clients, staff, and providers. Preferred qualifications include: One (1) year of nursing experience.
