Join a Leading MLTC Organization as a Remote RN Case Manager We are seeking highly skilled and compassionate Registered Nurses (RNs) to join our team as RN Case Managers. Key Responsibilities As an RN Case Manager, you will play a vital role in ensuring our members receive accessible and appropriate care. Your primary responsibilities will include: Managing a dedicated caseload within one borough (Bronx, Brooklyn, or Manhattan), ensuring members receive comprehensive care and support. Developing, overseeing, and updating individualized care plans to align with members' evolving needs and goals. Conducting home visits (4-6 visits per week) to assess home safety and care needs, identify potential risks, and develop strategies to mitigate them. Collaborating with leadership and Intensive Care Management teams to address complex cases, share best practices, and drive quality improvement initiatives. Presenting high-risk cases for multidisciplinary review, contributing to data-driven decision-making and care planning. Maintaining accurate and up-to-date records, ensuring compliance with regulatory requirements and organizational policies. Essential and Preferred Qualifications To be successful in this role, you will need: Active New York State RN License required. Bachelor's degree in Nursing, Health, or Human Services preferred. Minimum two (2) years of home care experience required, with a focus on care planning, patient assessments, and home safety evaluations. Strong experience in care planning, patient assessments, and home safety evaluations essential. Prior supervisory experience a plus. Bilingual candidates strongly preferred (see language preferences by borough below): Bronx: Bilingual Spanish preferred. Brooklyn: Bilingual Chinese, Spanish, or Russian preferred. Manhattan: Bilingual Cantonese or Mandarin preferred. Proficiency in Microsoft Office required. Skills and Competencies To excel in this role, you will need: Excellent communication and interpersonal skills, with the ability to build trust and rapport with members, families, and healthcare providers. Strong analytical and problem-solving skills, with the ability to assess complex cases and develop effective care plans. Ability to work independently and collaboratively as part of a multidisciplinary team.
