As a Case Management Analyst, you will be responsible for conducting comprehensive assessments of clients' needs, developing and implementing care plans, and collaborating with healthcare professionals to achieve optimal outcomes. This is a remote position, and you will have the flexibility to work from home while still being an integral part of our team. **Key Responsibilities** * Conduct comprehensive assessments of clients' needs, using care management instruments and data/information audits to identify areas of concern and develop strategies for improvement. * Develop and implement care plans that address clients' medical, social, and emotional needs, while also taking into account their individual goals and preferences. * Collaborate with healthcare professionals, including caseworkers, managers, and clinical leaders, to ensure that clients receive the best possible care and support. * Facilitate and coordinate care plan activities, including arranging appointments, scheduling services, and coordinating with healthcare providers. * Monitor and evaluate care plan progress, making adjustments as needed to ensure that clients are receiving the best possible care. * Identify and address high-risk factors and service needs that may impact client outcomes, and collaborate with healthcare professionals to develop strategies for mitigation. * Use case management and quality assurance processes to ensure compliance with regulatory and accreditation standards, as well as arenaflex policies and procedures. **Essential Qualifications** * Bachelor's degree in a related field, such as behavioral health, social work, or a related field. * 2+ years of experience in conduct health or social services. * 2+ years of experience with Microsoft Office applications, including Word, Excel, and Outlook. * Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments. * Excellent communication and interpersonal skills, with the ability to work effectively with clients, healthcare professionals, and other stakeholders. * Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork. **Preferred Qualifications** * Experience in case management and care coordination, including release planning and care planning. * Supervisory or leadership experience in a healthcare setting. * Education and certification in a related field, such as a master's degree in social work or a related field. **Skills and Competencies** * Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments. * Excellent communication and interpersonal skills, with the ability to work effectively with clients, healthcare professionals, and other stakeholders. * Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork. * Strong attention to detail and organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously. * Ability to maintain confidentiality and handle sensitive information with discretion. **Career Growth Opportunities and Learning Benefits** At arenaflex, we are committed to supporting the growth and development of our employees.
