Provide high-quality and compassionate care to Aetna members in the San Antonio, TX area. Use clinical expertise to assess, plan, implement, and coordinate healthcare services for members. Work collaboratively with other healthcare professionals to ensure the best possible outcomes for members. Conduct thorough assessments of members' medical needs and develop personalized care plans. Monitor and evaluate members' progress and adjust care plans as needed. Advocate for members' needs and ensure they receive appropriate and timely healthcare services. Maintain accurate and up-to-date documentation of members' care plans and progress. Communicate effectively with members, families, and healthcare providers to promote understanding and coordination of care. Stay current on industry trends and evidence-based practices to continuously improve care coordination. Adhere to all regulatory and compliance guidelines to ensure the highest quality of care for members. Participate in interdisciplinary team meetings to discuss and coordinate care for members. Collaborate with community resources to help meet members' social, emotional, and physical needs. Continuously strive to improve healthcare outcomes for members and identify opportunities for process improvement. Serve as a role model and mentor for other healthcare professionals.
