Care Coordinator - Remote (Part Time)

Job Details: Under the minimal supervision, this position will serve as the coordinator for care of onboarding patients. Responsible for researching, obtaining, and coordination of financial assistance and copay assistance for specialty and infusion patients. This may include verification of insurance coverage, entering/updating patient information into the computer system, communication of financially responsibility and securing financial assistance. Additional responsibilities include orientation and mentoring inexperienced staff and daily oversight of team members. Also, monitoring the performance of the Care Coordinator Advocate team and create and update policies and procedures. Responsibilities • Maintain complete, organized, and accurate files for all patients relating to coordination of services: case management, nursing services, pharmacy preparation and delivery, reimbursement authorization and any information/data significant to the overall patient care coordination for patients. • Responsible for the input of missing data into computer system on all patients and prior documentation relating to care coordination. Informational data should be from the point of patient notification through verification of benefits, initiation of care, first day of therapy follow-up, completion of services, discharge and cessation of communications and re-authorizations as assigned • Strict adherence to all Policies & Procedures ensuring that care coordination of patient therapy and staff involved in their respective processes are complying with stated guidelines and maintaining current working knowledge of all areas. • Identification and resolution of all care coordination complaints, concerns or deficiencies with written documentation being given to the Department Manager. • Professional and courteous interface with internal and external personnel participating in patient care coordination. This would include, but not be limited to hospital discharge planners, home health agency staff, physicians, physician office personnel, case managers, health benefit providers, hospital employees, center team members and all other referral sources. • Participation in general and departmental specific orientation/training of new center staff to include overall referral process, computer and paperwork documentation, cross-training, and work duties. • Assist in interview process when recruiting new hires. • Performs in accordance with system-wide competencies/behaviors. • Performs other duties as assigned. Skills & Abilities • Proficient computer skills in Windows, Excel, and other related software • Knowledge of medical terminology • Strong verbal and written communication skills • Ability to work collaboratively among multiple departments • Excellent interpersonal skills with the ability to collaborate with varying contacts and organizational levels to maintain all efforts of patient advocacy • Excellent customer service • Ability to demonstrate organizational skills in a fast-paced, ever-changing environment • Home Infusion pharmacy knowledge preferred Requirements • High School diploma or GED CarepathRx offers a comprehensive benefit package for full-time employees that includes medical/dental/vision, flexible spending, company-paid life insurance and short-term disability as well as voluntary benefits, 401(k), Paid Time Off and paid holidays.
